Whether the prognostic impact of intermediate-risk genetic abnormalities in adult acute lymphoblastic leukemia (ALL) and acute myeloid leukemia (AML) is similar to that reported for pediatric ALL, AML, and chronic lymphocytic leukemia (CLL) is incompletely understood. We retrospectively analyzed gene mutations in 2346 adults with ALL (n = 1326), AML (n = 1024), or CLL (n = 516) with FISH data. The median follow-up was 47 months. There were 1065 cases of intermediate-risk ALL or AML. In adults, there were more high-risk abnormalities (P < 0.001), older age (P = 0.004), and an intermediate-risk lifestyle (smoking, alcohol, geographic >150 miles) (P < 0.001). Analysis of the incidence of intermediate-risk genetic abnormalities in ALL patients with intermediate-risk genetics suggests that the incidence of abnormal genetic abnormalities in these patients parallels that of high-risk patients. Clinically, only 43% of intermediate-risk patients achieved complete remission (CR), as compared with 72% of high-risk patients. In adult AML with intermediate-risk genetics, there was no difference in CR rate (80%) compared with high-risk patients (83%), and the median progression-free survival (PFS) was 19 months vs 32 months (P < 0.001) and 10 months vs 12 months (P = 0. d2c66b5586
